Etiquette

Please make sure to turn off noises made by electronic devices and refrain from bringing a plus-one or children to your appointment. Inappropriate behavior and/or conversation is never tolerated. I require full, appropriate draping with no exceptions.

Appointments

It is recommended that you book in advance to ensure that you get your preferred time and date. My schedule books out a year in advance. Services are non-refundable. Packages and gift certificates are non-refundable. A 24 hour notice is required to cancel or reschedule an appointment. This allows someone else the opportunity to take that appointment in your place. The 50% deposit you make on your appointment is non-refundable and will be forfeited should you fail to cancel/reschedule your appointment within the 24 hour timeframe. If a deposit is not made, you will be sent an invoice for 50% of your appointment value. If the invoice is not paid within the given timeframe, you will not be allowed to book again.

Cancellation Policy

A 24 hour notice is required for appointment cancellations. Any cancellations with a less than 24 hour notice will forfeit their 50% deposit. Those who have not made a deposit will be sent an invoice for the appropriate monetary amount. If the invoice is not paid, the client will not be allowed to book again. Those who habitually cancel and/or reschedule will not be permitted to book again. Gift certificates and packages may also be forfeited due to late cancellations.

No Shows

Clients who fail to arrive to their appointments will forfeit their 50% deposit. Clients paying via gift certificate or package who no-show their appointments will forfeit their gift certificate or package. Clients who have pre-paid for appointments and fail to arrive to their appointments will forfeit that pre-paid appointment. If no pre-payment was made, you will be invoiced. Those who fail to pay their invoice will not be allowed to book again.

Arrival Time

Please arrive about 5-10 minutes prior to your appointment time to allow for check in and intake forms. If you arrive late, we may need to complete your service in the time frame that was originally scheduled. You will be charged for the full appointment time.

Gift Certificates

Gift certificates can be purchased online or in person. Gift certificates expire one year after the date of purchase. Gift certificates are non-refundable. Gift certificates will be forfeited if the appointment is cancelled with less than 24 hour notice.

Packages

Packages can be purchased online or in person. Packages expire one year after the date of purchase. Packages are non-refundable, but may be transferred or shared with friends or family. Packages will be forfeited if the appointment is cancelled with less than 24 hour notice.

Intake Forms

All intake forms are kept private. Your personal information is never shared. By booking your appointment, you agree to the following: I have completed the proper form(s) to the best of my ability and knowledge and agree to inform my therapist if any of the above information changes at any time. I understand that communication is key and will let my therapist know if ever I am uncomfortable, pressure is too deep or too light. I understand that appropriate draping is required. I understand that female breasts are never touched. I understand that genitals are never touched. I understand that inappropriate behavior is never tolerated. I understand that I may end the session at any time. I agree to cancel my appointment 24 hours in advance if the need arises. Massage therapists do not diagnose any diseases. Massage  is not a replacement for medical care. I voluntarily assume reasonable risk, consent to treatment, and hold harmless BH Bodywork, LLC for any claims of illness, injury, or economic loss. If person is a minor, their parent/legal guardian agrees to these policies.
